The KSPC team is dedicated to ensuring that your first visit with our practice starts you on a path to pain relief.
To make your appointment as successful as possible, please read the following so you arrive prepared and knowing what to expect.
What to bring
Completed New Patient Paperwork - Click here
A list of the medications you take including prescription medicines, nonprescription medicines, vitamins, and supplements, the dose and how often you take each
Request your primary care provider to submit a referral which includes any recent X-rays, CT scans, or MRIs you might have related to the problem for your pain specialist to review
A list of questions or concerns to discuss with your provider
What to expect
We understand how pain can affect your life and the lives of those you love. As a patient of our practice, our mission is to provide successful treatment options so that you can overcome the challenges you face by living in pain and get back to the life you deserve.
During your first visit, you will receive a thorough evaluation that includes your medical history and a physical examination pertinent to your pain problem. This comprehensive visit with your pain specialist will last approximately 30 minutes. Depending on your condition, we may recommend a combination of procedures and/or therapies to achieve the optimal results. We believe in educating our patients and empowering them with the ability to decrease their pain.
Billing Inquiries
As a courtesy, we will bill your insurance carrier on your behalf, and only collect the amount deemed your responsibility from your insurance carrier. This will include payment for any co-payment at the time of service and for any deductible or non-covered services determined by your contract with your insurance.
We recommend that you verify your insurance benefits prior to appointment so that you will have a a full understanding of your covered benefits, limitations, and financial commitment. We accept payment via check, American Express, Visa, Master Card, Discover, or cash.
